In consideration of my being permitted to receive instruction from, Durastudio LLC dba UCB Program, the SERVICE PROVIDER, I expressly waive and release SERVICE PROVIDER from any and all liability that may arise from my participation in SERVICE PROVIDER activities. This includes, but is not limited to, any injuries received from participation, instruction or transportation associated with SERVICE PROVIDER activities.

Concerning my health, I state the following:

I do not have any physical conditions, limitations or injuries that would prevent my participation in any activities.
If under care of a physician, I have discussed participation with my physician and have received a release.

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.

By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public educational setting.
